VLT Consent Form and Terms

Please complete this consent form before attending the Virtual Language Tables’ sessions for the first time. Osaka Metropolitan University (OMU), Osaka City University (OCU), and Osaka Prefecture University (OPU) may use screen capture images, video recordings, audio materials, and contents from the Virtual Language Tables (VLT) for social media, websites, publications (newsletters, articles, conference presentations, etc.) for educational purposes; All participants will be kept anonymous unless they give us their permission. If you do not want your image, video recordings, audio recordings, and/or content to appear in the materials they use, please select “No” so that they can make sure you do not appear in the materials. We would appreciate it if you would permit us to use any of your images, video recordings, audio recordings, and contents in order to improve our programs. Personal information collected at the VLT will be managed properly and will not be utilized for any purposes other than the operation of the COIL Division’s programs including VLT. If collected information is utilized for research purposes, it will be handled in a form that ensures individuals cannot be identified and will be kept strictly confidential. If you participate in the VLT without submitting this form, it will be considered that you have agreed to their use of your images, video recordings, audio recordings and contents.
